Using DCCR to convey clinical information

Interim arrangement until eCare goes live in April 2027

A note in the record is not a referral. To refer a medical patient, phone the Acute GP service on 01872 226263. Out of hours, phone the medical registrar, or the Silver Phone for frailty. Surgical specialties take referrals by phone directly.

How hospital teams see your notes

Clinicians in ED, MRATS and SDMA routinely check the Devon & Cornwall Care Record (DCCR) for medical patients. DCCR shows:

  • current medications

  • past medical history

  • Treatment Escalation Plan (TEP) documentation

  • the most recent clinical entries from the GP record

What to do when you refer

  1. Write your reasons for admission in the consultation record itself. Include your main concerns, the question you want answered, and anything the team won't know: recent trends, results, discussions with family, and ceilings of care. Separate letters and attachments do not appear in DCCR.

  2. Tell the Acute GP service if there's something the team must read. They will add "See DCCR" to the On Take Register entry.

  3. Remember that only recent entries show. Blood results, accuRx messages or admin notes added after your referral can push your consultation note out of view. If that's likely, please give the key points to the Acute GP service on the phone so they go on the On Take Register.

Special situations

  • Patients who have opted out of DCCR: give the clinical details to the Acute GP service when you refer, so they can record them on the On Take Register. If more information is needed, send a copy with the patient.

  • Home visits, or when you can't reach the record: the Acute GP service will document the referral for you over the phone​
