Intermediate Care

This is a 24hr/7 days a week service, located at Longreach, for SWAST & primary care clinicians to refer into.

They serve patients in crisis with no acute medical need, an acute mental health-related risk which you feel isn't safe at home .

The Assessment Hub team will call the patient directly if they have capacity to see.
