The following is a community pathway for Emergency Care Practitioners (ECPs) to follow for access to further management. The pathway is operational 0900 – 1700 every day - direct admission (the middle spoke of the diagram below) is NOT available outside these hours, ECPs should not contact the hospital juniors to request direct admission out of hours

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

Notes for Acute GP

  • All patients accepted for Acute GP clinic or UCC assessment must have transport home

  • Patients must be discussed with the Acute GP before they can be accepted for Acute GP clinic, or UCC at Treliske or WCH

 

 

Potential Ambulatory Care Sensitive Conditions

  • Pulmonary embolism

  • Pneumothorax

  • Pleural effusion

  • Asthma

  • Infective exacerbations of COPD

  • Community acquired pneumonia/LRTI

  • Congestive Cardiac Failure

  • Atypical sounding Chest pain

  • Crescendo TIA

  • Acute Headache (systemically well)

  • Upper gastrointestinal Haemorrhage

  • Gastroenteritis

  • Painless obstructive Jaundice

  • Anaemia

  • Hypoglycaemia

  • Potential diabetic complications type 1 and type 2

  • Cellulitis

  • PEG associated problems

  • Oesophageal obstruction (known stented or unstented)

  • Acute admission from Care Homes.

  • Falls including syncope or collapse

  • End of life care

  • Urinary tract infections/pyelonephritis

  • Deliberate Self harm
