Coronavirus: Changes to Pathway for Older People living with Frailty needing assessment.

Community Beds

​

In hours (0800-2000hrs)

​

All admission can currently be made to community beds via CATU units (via AGP unless WCH)

  • WCH: accepting from own locality 24/7 7 days per week

  • CRCH: accepting from own locality 08:00 – 20:00 hrs 7 Days per week 

  • BCH: accepting from own locality 08:00 – 20:00 hrs 7 Days per week

Ensure an accompanying:

  • TEP 

  • patient profile

  • (and EDF where appropriate)

Out of hours (2000 – 0800hrs)

​

Non-urgent: Referrers need to fill out a SERF form. WCH is able to accept without a SERF.

 

Urgent referral RCH will be accepting patients whose needs cannot be met at home from both Central and North & East ICAs, whilst WCH will accept patients out of hours for West ICA until such time all OPAL units are able to provide a 24/7 service. 

 

RCH (Treliske site) 

Becomes the admitting point for older people for escalation only; ie. those with an ‘acute’ need for bedded care and interventions that can only be provided at RCH ie. NIV, ventilation, dialysis, acute stroke treatment and therapy.

All medically optimised patients who require further bedded care in the community or require support at home must follow the SERF form process. 

​

Single Electronic Referral System (SERS)

If you cannot complete the form electronically you can call 07584 334473 where a member of the Integrated Urgent Care Service will complete the form with you. This is available between the hours of 08.00 and 20.00 This will replace all referral forms (for example eBICA) into community services, except for children's services, mental health and primary care.
